City Clerk - Birth Records
Illinois schools require a certified birth record when
registering children for school for the first time in kindergarten
or first grade, or students moving to Illinois from out-of -state. For children born at Lake Forest Hospital within the last ten
years, a certified birth record may be obtained from the City
Clerk’s office in person at City Hall. Applications
for Certified Birth Certificates (6k - 1 page)
can be found at the front counter. Office hours: 8:00 a.m. to
12:00 noon and 1:00 p.m. to 4:30 p.m. Monday through Friday.
You may also apply by mail addressing your request to: Registrar,
220 E. Deerpath, Lake Forest 60045. Please include name listed on
birth record, birth date, the mother’s and father’s full
names, the mother’s maiden name, person requesting record,
number of copies requested, a photocopy of your driver's
license or photo ID with signature.
The cost is $10.00 for the first copy and
$4.00 for additional copies bought at the same time. Make checks
payable to "The City of Lake Forest." When requesting
birth certificate copies by mail, please include a stamped,
self-addressed envelope. Once your request is
received, the certified copies are mailed out within four days of
receipt. For

For individuals over age 10, you may obtain copies from the
County Clerk’s Office, 18 N. County, Waukegan. Office hours: 8:30
a.m. to 5:00 p.m. Monday through Thursday, 8:30 a.m. to 7:30 p.m. on

Birth records are restricted in Illinois and may be obtained
only by the parent or legal guardian of a child, the person named on
the record if he/she is 18 or older, or a legal representative
presenting notarized authority to obtain the record. |
