City Clerk - Death Certificates

A certified death certificate record may be obtained from the City Clerk's office in person at City Hall.  An Application for Certified Death Certificatepdf(3k - 1 page) can be found at the front counter. Office hours: 8:00 a.m. to 12:00 noon and 1:00 p.m. to 4:30 p.m. Monday through Friday.

You may also apply by mail addressing your request to: Registrar, 220 E. Deerpath, Lake Forest 60045. Please include name listed on death record, date of death, person requesting record, number of copies requested, intended use for obtaining record, a photocopy of your driver's license or photo ID with signature, as well as a correct return address of where you want the copies mailed to.

The cost is $12.00 for the first copy and $4.00 for additional copies bought at the same time. Make checks payable to "The City of Lake Forest." Once your request is received, the certified copies are mailed out within 72 hours. For more information, call 847-810-3676.

You may obtain copies from the County Clerk's Office, 18 N. County, Waukegan. Office hours: 8:30 a.m. to 5:00 p.m. Monday through Thursday, 8:30 a.m. to 7:30 p.m. on Friday. For more information, call 847-377-2411.
