City Clerk - Death Certificates
A certified death certificate record may be obtained from the
City Clerk's office in person at City Hall.
An Application for Certified
Death Certificate (3k - 1 page)
can be found at the front counter. Office hours: 8:00 a.m.
to 12:00 noon and 1:00 p.m. to 4:30 p.m. Monday through Friday.
You may also apply by mail addressing your request to: Registrar,
220 E. Deerpath, Lake Forest 60045. Please include name listed on
death record, date of death, person requesting record, number
of copies requested, intended use for obtaining record, a
photocopy of your driver's license or photo ID with signature,
as well as a correct
return address of where you want the copies mailed to.
The cost is
$12.00 for the first copy and $4.00 for additional copies bought at
the same time. Make checks payable to "The City of Lake
Forest." Once your request is received, the certified copies
are mailed out within 72 hours. For more information, call

You may obtain copies from the County Clerk's Office, 18 N.
County, Waukegan. Office hours: 8:30 a.m. to 5:00 p.m. Monday
through Thursday, 8:30 a.m. to 7:30 p.m. on Friday. For more
