Any Lake Forest Police Department member will take your information.  Please provide your name and contact information, the department member's name, type of incident or concern, date, and time of incident.  The information will be forwarded to the department member's supervisor.  Department supervisors will appropriately disseminate your compliment or investigate your complaint.

Effective January 1, 2004: Illinois Law, Public Act 93-0592, requires: Anyone filing a complaint against a sworn peace officer must have the complaint supported by a sworn affidavit.
