Illinois schools require a certified birth record when registering children for school for the first time in kindergarten or first grade, or students moving to Illinois from out-of-state. For children born at Lake Forest Hospital, a certified birth record may be obtained in person at City Hall. Applications for Certified Birth Certificates can be found at the front counter. Office hours: 8:00 a.m. to 12:00 noon and 1:00 p.m. to 4:00 p.m. Monday through Friday.
You may also apply by mail addressing your request to: Registrar, 220 E. Deerpath, Lake Forest 60045. Please include name on birth record, birth date, the mother’s and father’s full names, the mother’s maiden name, person requesting record, number of copies requested, a photocopy of your driver's license or photo ID with signature. Please be sure to include your telephone number.
The cost is $10.00 for the first copy and $4.00 for additional copies bought at the same time. Make checks payable to "The City of Lake Forest." For more information, call 847-810-3676.

Birth records are restricted in Illinois and may be obtained only by the parent or legal guardian of a child, the person named on the record or a legal representative presenting notarized authority to obtain the record.
