During the COVID-19 public health crisis, in-person transactions for vital records requests at City Hall has been temporarily suspended.
You may obtain a Certified Death Record by mailing in an Application for Certified Death Certificate to:

Please include name listed on death record, date of death, person requesting record, number of copies requested, intended use for obtaining record, a photocopy of your driver's license or photo ID with signature, as well as a correct return address of where you want the copies mailed to.
The cost is $14.00 for the first certificate and $6.00 for each additional certificate. Make checks payable to "The City of Lake Forest." For more information, call 847-810-3676.
Death records are not public records and are only available to those who have a personal or property right interest with the decedent. If you are not a relative of the deceased person, a letter or document from the office or agency that needs the death certificate must accompany the request.
