How do I obtain a copy of my personal medical records?
- Patients may visit the Public Safety Building to obtain a copy of their EMS records.
- A valid photo ID must be presented.
- Please call 847-810-3867 ahead to coordinate a time.

How do I obtain a copy of medical records for a family member?

- Visit the Public Safety Building to obtain a copy of EMS records.
- A valid phot ID must be presented.
- Please call 847-810-3867 ahead to coordinate a time.
- If you are the legal authorized representative of a patient, valid proof is required (e.g., medical power of attorney).
- If the patient is living but incapacitated with no authorized representative, valid proof of patient’s state is required, however, release of records is not guaranteed.
- If the patient is deceased and you are not the decedent's appointed representative, valid authorization for the release of records signed by the appointed representative of the decedent's estate in accordance with (735 ILCS 5/8-2001.5) is required. Proof of appointment as representative of the decedent's estate must also be provided.
- If the patient is deceased with no appointed representative to the decedent's estate, an authorized relative in a accordance to (735 ILCS 5/8-2001.5) may complete the Authorized Relative Certification to obtain patient records.
