Please include name listed on death record, date of death, person requesting record, number of copies requested, intended use for obtaining record, a photocopy of your driver's license or photo ID with signature, as well as a correct return address of where you want the copies mailed to.
The cost is $14.00 for the first certificate and $6.00 for each additional certificate. For mail-in requests, there is a $2.20 additional charge for postage. Make checks payable to "The City of Lake Forest." The table below describes common amounts requested and the cost rule will apply for any amount of copies past and in between these amounts. Death records are not public records and are only available to those who have a personal or property right interest with the decedent. If you are not a relative of the deceased person, a letter or document from the office or agency that needs the death certificate must accompany the request.
